Alström sendromu karaciğer sirozunun oldukça nadir bir sebebidir. Alström sendromlu hastaların post-mortem biyopsilerinde karaciğer, böbrek, kalp ve akciğerler gibi birçok organda fibro- sis gösterilmiştir. Variceal development is the most important portal hypertension related complication. Varices most commonly occur around the gastroesophageal junction, but ectopic varices may develop in many gastrointestinal and extra-gastrointestinal localizations. Duodenum is one of the most common localizations for ectopic varices. Diagnosis of duodenal varices is usually made by upper gastrointestinal endoscopy, but endoscopic appearance is not diagnostic and usually further investigations are required in order to make accurate diagnosis. Endoscopic ultrasound is the gold standard method for the examination of gastrointestinal submucosal lesions therefore it is alsouseful in the work up of suspected duodenal varices. Here we present a patient with cryptogenic liver cirrhosis followed in our clinic, whom duodenal lesions suspected of duodenal varices were noticed during upper gastrointestinal endoscopic examination and endoscopic ultrasound was used to confirm the presence of duodenal varices.Article Effect of Local Site Classes on Torsional Irregularity in Structures According To Turkish Seismic Codes 2007 and 2018(Konya Technical University, 2020) Çetin, Kabil; Demir, Ali; Altıok, Taha YasinA major parts of Turkey is located on the active seismic belts. Until now, many structureshave been damaged and even collapsed due to the destructive effects of the earthquake. The earthquakeshave caused largely in loss of life and property. The inevitable earthquake fact has revealed the concept ofearthquake resistant design. The structures must be had sufficient rigidity and strength according toearthquake resistant structure design principles. Load-bearing system and the distribution of these twostructural properties must be formed symmetrically and regularly. However, irregular structures areconstructed due to architectural reasons. In these structures which are irregular in terms of structuregeometry and distribution of rigidity, is seen torsional irregularity. Torsional irregularity negatively affects the earthquake performance of the structure. Therefore, it becomes a subject that needs to be meticulouslyinvestigated.In this study, effect of different local site classes on torsion irregularity in structures have been investigatedaccording to Turkish Seismic Code-2007 and Turkish Building Earthquake Code-2018. Modeling andearthquake analyses have been performed in SAP 2000 program. Earthquake analyses of 270 structures for5 different structure types have been carried out by using the Equivalent Seismic Load Method and ModeSuperposition Method. Torsional irregularity coefficients of the structures on different local site classeshave been obtained for 4, 6 and 8-storey structures. The results have been compared according to bothearthquake analysis methods and Turkish Seismic Codes. In the Turkish Building Earthquake Code-2018,torsional irregularity coefficients of the regular structures have decreased compared to the Turkish SeismicCode-2007, while there has been seen a substantial increase in torsional irregularity coefficients of theirregular structures. The effect of local site classes on the calculations has become more realistic along withthe Turkish Building Earthquake Code-2018. It has been concluded that local site class has a significanteffect on torsional behavior in Turkish Building Earthquake Code-2018 and this effect must be taken intoconsideration in the design and evaluation of the structures with torsional irregularity.
